Everyone said a small software team needed more discipline. The real problem was an operating model that concentrated all the project risk at the end, on the people least able to carry it.

A support team was blamed for underperforming. Mapping the actual flow of work revealed something else: a process set up to fail, and people paying the price for it.

A consultancy imposed the right framework on an organisation that wasn’t ready for it. I got the redesign right, then learned the harder lesson: moving in the same direction isn’t the same as bringing people with you.
